Wording for a Notice of Hearing and Certificate of Service on a Motion to Dismiss Hearing? - thank you certificate wording

Please can someone explain the correct wording for a:

Notice of hearing

Service Certificate

on a motion to dismiss the hearing?

If you are familiar with the title of a document, in the case, then it would follow the same format for the header that follow detail the number of cases decided by the Court to inform the plaintiff and the defendant to information.

The title is as follows: NOTICE OF HEARING ON request for dismissal

In paragraph (area under the title above), you can write: Please note that [you - Make you move ???], the defendant to dismiss the complaint, as suggested accompaniment. Hearing is for the [Date and time] [place - should not] the same court.

Certificate of service is like any other (as you said they were familiar with them).

Attach the original motion and send it by mail, fax, etc. (what service standards are in your state), but I wanted to do on all service methods.
